                   Small Group Plans

Small group health insurance plans are defined as lesser
than 50 lives, in some states small group plans are
community rated, in some states are age/gender rates.
In the state of Illinois, a maximum of 67% rate up can be
    assessed due to medical conditions.

All of our group plans include plans with flexible options,
varying levels of required employer contributions and
ample provider networks, all working to satisfy employee
health coverage needs while controlling employer’s
    related costs.

Small Group Health Insurance plan requirements:

  • The employer must have at least two full-time employees.
  • Proof of full-time employment is required.  For
documentation, most insurers accept the employer's last
quarterly wage/tax report submitted to the state
    unemployment insurance department.
  • Most group health insurers require at least 75% of full-time
employees to either (1) enroll in the group health plan or
    (2) participate in their spouse's group health plan.  In
addition, most insurers require at least 50% of full-time
employees to enroll in the group health insurance plan.
  • Most health insurers require the employer to contribute
at least 50% of the premium for employee health coverage,
through they do not require the employer to contribute
toward the cost of dependent coverage.
