major, minor at 80/20 cost share, and major at 50/50 cost-

share. (Orthodontics optional)Who should pay: Employer

is required by most insurance companies to pay a

minimum of 50% of the employee's cost. Employers can

pay 100% of employee and dependent costs. Participation

requirements are 75% for eligible employees and 75% of
